On anesthesia coverage, new rules in some states prohibit insurers from imposing time limits on anesthesia when recommended by a medical professional, requiring coverage to extend for the entirety of a procedure rather than being cut off at a predetermined time threshold.
Other 2026 state-level changes noted in the report include new prior authorization restrictions for pediatric hospital transfers in at least one state and continued scrutiny of Medicare Advantage risk adjustment practices at the federal level.
