Plans are now required to inform both enrollees and providers of coverage decisions whenever a request is submitted on a patient's behalf, and enrollee financial liability generally cannot be finalized until the plan issues a payment decision on the underlying claim.

The changes arrive alongside other 2026 Medicare Advantage provisions establishing new guardrails on special supplemental benefits for chronically ill members and updated risk adjustment data submission requirements, according to the same report.
